Job Description:

  • Other duties as assigned.
  • Communicates with patients and families after discharge to address concerns and ensure understanding of follow-up care and functional assessment.
  • Collaborates with the multidisciplinary team to ensure high-quality patient care.
  • Participates in community education as needed. Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner.
  • Provides telephone and face-to-face consultations with patients, answering questions, providing educational information, and reviewing discharge information.

Effective verbal and written communication skills [Required]
Ability to read, write and explain moderately complex information to hospital staff with a great deal of attention to detail [Required]
Ability to communicate effectively in a supportive manner to patients, family, staff and physicians [Required]
Ability to explain information to patients and families in a way the patient understands, and/or explain departmental or hospital procedures to employees, visitors and patients [Required]
Ability to observe assess, guide and evaluate others [Required]
Ability to communicate timely and in an open and honest manner [Required]
Ability to collect data, interpret findings, set priorities and carry out established plans [Required]

Ability to work proactively and handle multiple tasks simultaneously [Preferred]
