Job Description:

  • Manages legal issues related to adoptions, guardianship, and advance directives in collaboration with legal counsel.
  • Educates patients and families on available resources and coping strategies for dealing with diagnosis, treatment, and prognosis.
  • Collaborates with hospital and community agencies to secure needed services and resources for patients and caregivers.
  • Coordinates with RN Case Managers to develop and implement post-discharge plans tailored to each patient's needs.
  • Interviews patients and families to assess psycho-social needs and evaluate support systems.
  • Collaborates with the healthcare interdisciplinary team to address barriers impacting discharge plans.
  • Identifies and initiates the most efficient discharge plan to expedite patient transition to the next level of care.
  • Facilitates communication between patients, families, and healthcare providers to ensure understanding of discharge plans and available resources.
